Introduction
The ย purpose ย of the ย transition ย planning ย and ย support ย process ย is to provide ย overall planning for service transitions and to coordinate the resources that they require.
The Objectives are:
โ€ขย ย ย ย ย ย ย ย ย ย ย ย ย  Plan and coordinate the resources to ensure that the requirements of service strategyย  ย encodedย  ย in ย serviceย  ย designย  ย areย ย  effectivelyย  ย realizedย  ย in ย service operation.
โ€ขย ย ย ย ย ย ย ย ย ย ย ย ย  Coordinate ย activities ย across ย projects, ย suppliers ย and ย service ย teams ย where required.
โ€ขย ย ย ย ย ย ย ย ย ย ย ย ย  Establish ย new or changed ย services ย into supported ย environments ย withinย  the predicted cost, quality and time estimates.
โ€ขย ย ย ย ย ย ย ย ย ย ย ย ย  Establish ย new ย or ย modified ย managementย  ย informationย  ย systems ย and ย tools, technology and managementย  architectures, ย service management ย processes, andย  measurement ย methods ย and ย metrics ย to meet ย requirements ย established during the service design stage of the lifecycle.
โ€ขย ย ย ย ย ย ย ย ย ย ย ย ย  Ensureย  that all parties adopt the common ย framework ย of standard ย re-usable processes and supporting systems in orderto improve the effectiveness and efficiency of the integrated planning and coordination activities.
โ€ขย ย ย ย ย ย ย ย ย ย ย ย ย  Provide clear and comprehensive ย plansthat enable customer and business change projects to align their activities with the service transition plans.
โ€ขย ย ย ย ย ย ย ย ย ย ย ย ย  Identify, ย manage ย and ย control ย risks, ย to ย minimize ย the ย chance ย of ย failure ย and disruption across transition activities; and ensure that service transition issues, risks and deviations are reportedto the appropriate stakeholders and decision makers.
