IMPORTANT REMINDERS:   

  • PLEASE  CHECK THE DATE  AND  TIME  OF  RELEASE. BRING OLD PASSPORT WHEN CLAIMING NEW PASSPORT.
  • Paspports unclaimed after six (6) months from the date of issuance will be cancelled, per Department Order No. 37-2003.
  • Upon receipt of the new passport, please transfer immediately the visa to the new passport to avoid payment of immigration fines.
  • The Philippine Embassy no longer extends the validity of Philippine passports, without exemptions. This follows a directive from the Department of Foreign Affairs (DFA) to comply with International Civil Aviation Organization (ICAO) regulations and the Chicago Convention.
  • The public is advised to apply for passport renewal at the Embassy seven (7) months to one (1) year before the expiry date on the passport data page.
  • If the visa will expire earlier than the passport, the public is advised to apply for passport renewal three (3) months before the visa’s expiry date.

Requirements for Passport Renewal (KD 19.50 for renewal)

  • Personal appearance
  • Duly filled-out passport application form
  • Original passport
  • Photocopy of passport data page and last page
  • Photocopy of Kuwait residence visa, if any
  • Those whose passports are about to expire within the next six (6) months, or whose Kuwait residence visas are about to expire within one year from the date of expiry of the passport, should apply for RENEWAL of their passport in order to avoid any lapse in the validity of their passports and payment of immigration penalty
  • Use BLACK INK in filling up the passport application form
  • Passports that have expired for more than one year may no longer be extended
  • The validity of passports may only be extended once
  • Upon receiving the new passport, the Kuwait residence visa must be IMMEDIATELY TRANSFERRED to avoid immigration penalties


Requirements for Issuance of New Passport and Report of Birth for Minors (KD 
19.50 for new passport; KD 8.125 for Report of Birth)

  • Personal appearance
  • Duly filled-out passport application form
  • Duly filled-out Report of Birth form (4 original copies)
  • Arabic birth certificate of the child with authentication from the Ministry of Foreign Affairs and official English translation (plus four photocopies).
  • Marriage certificate of the parents (DFA-authenticated PSA/NSO Marriage Certificate or Report of Marriage) (plus four photocopies)
  • Passport and Civil ID, if any, of the parents (plus four photocopies).


Requirements for Replacement of Lost Passport
 (KD 48.750 if lost ePassport; KD 29.250  if lost green passport or MRP)

  • Personal appearance
  • Duly accomplished passport Application Form
  • Police Report with English translation
  • Affidavit of Loss
  • If possible, photocopy of the lost passport/Kuwait Civil ID


For woman who wants to adopt her married name

  • Marriage Certificate in SECPA (security paper) from PSA/NSO-authenticated by the DFA in Manila
  • If the marriage was solemnized by the Kuwait Ministry of Justice, the Marriage Certificate (with English translation) must be
    authenticated by the Ministry of Foreign Affairs and duly registered at the Philippine Embassy
  • Photocopy of old passport


Requirements for Solemnization of Marriages between Filipinos only
 (KD 52.00)

  • Photocopies of applicant’s passports
  • Birth Certificates issued by the PSA/NSO and authenticated by Authentication Division of the DFA
  • PSA/NSO Certificate re: non-appearance of the applicant’s name in the National Indices of Marriage or certificate of Single Status and Non-marriage issued by the PSA/NSO and authenticated by the DFA
  • Joint Affidavit of Single Status (form available)
  • Application for Marriage License (form available)
  • Notarized Affidavit of Parental Consent (if either applicant is 18-21 years old)
  • Notarized Affidavit of Parental advice (If either applicant is between 21-25 years old). Parent/s of the applicant should inform the Embassy in writing. If such advice is not obtained or is unfavorable, the marriage license shall be issued three (3) months following the publication of the application of the marriage license
  • Certificate of Marriage Counseling (If either applicant is 18-25 years old)
  • Two (2) pictures of each applicant
  • Notice of Marriage to be posted for ten (10) days

Report of Marriage (KD 16.250)

  • Duly filled-out Report of Marriage form
  • Arabic marriage certificate with authentication from the Ministry of Foreign Affairs and official English translation (plus 4 photocopies)
  • Passport and Civil ID, if any, of the spouses (plus four photocopies)

Notarization of Affidavits / Contracts (KD 8.125 per document)

  • Duly prepared and signed affidavits and contracts to be notarized
  • Copy of passport and Civil ID, if any, of the affiant/ signing party to the contract
  • Copy of passport and Civil ID, if any, of two (2) witnesses
  • Only the signatures of those who are physically present in Kuwait may be recognized by the Embassy for notarization purposes
  • In order to be notarized, the contents of the contracts or affidavits should not be contrary to law, good morals, customs or public policy
  • For notarization of Special Power of Attorney, the specific acts authorized and over which properties, should be indicated
  • For Special Power of Attorney that were drafted in the Philippines, please ensure that the blanks are completely filled up and the specific acts authorized and over which properties are indicated

Affidavits of Support and Consent (KD 8.125)

  • Duly filled-out Affidavit of Support and Consent
  • Photocopy of the Kuwait visa with official English translation
  • Photocopy of the passport and residence visa of the sponsor
  • Photocopy of the passport of the person invited

In order to prevent illegal recruitment and human trafficking, the Embassy has stopped notarizing Affidavits of Support and Consent for Commercial Visa except: (1) for Filipinos hired as private staff of diplomats; (2) for Filipinos sponsored by Kuwait Government; (3) for Filipinos taking professional accreditation examination in Kuwait; (4) for Filipinos attending Ministry of Labor-approved seminars or training; or (5) for Filipinos working in the oil field with three-month contracts.


Authentication of PSA/NSO Documents/Diploma/Transcript of Records/NBI Clearance (Yellow)/Notarized Documents in the Philippines (
KD 8.125 per document)

  • DFA-authenticated birth certificate/ marriage certificate/ diploma/ transcript of records/ NBI clearance/ notarized documents in the Philippines
  • Official Arabic translation of the document
  • Copy of passport and Civil ID, if any, of the applicant
  • Falsification of documents is a crime under Philippine laws and Kuwait laws
  • Falsified documents submitted to the Embassy shall be CONFISCATED outright and the names of the persons who submitted the said documents shall be REPORTED to the proper authorities for investigation
  • Anyone who submits a falsified document is presumed as the one who committed the falsification or is in connivance with the falsifier
  • Kuwait authorities require NBI clearances, diplomas and transcripts of records to be authenticated by the Kuwait Embassy in Manila after the DFA authentication
  • For documents notarized in the Philippines, a Certificate of Notarial Act from the Executive Judge must first be obtained in order for DFA to authenticate the document
  • Only DFA authentication certificates that have been issued within the last two years and which remain unspoiled, untampered and are not soiled or worn out will be accepted for Embassy authentication

Parental Travel Permits (KD 8.125)

  • Duly filled-out Parental Travel Permit
  • Photocopies of the passports of the signing parent, of the person accompanying the child and of the child
  • Signed by the parent who will not be accompanying the child

Certification of those Applying for Dependent Visa (KD 8.125)

  • Duly filled-out Certification
  • DFA-authenticated PSA/NSO birth certificate or marriage certificate
  • Photocopy of the passport and residence visa of the sponsor
  • Photocopy of the passport of the person invited

Certifications of Philippine Driver’s Licenses (KD 8.125)

  • Original License
  • Copy of passport of the applicant
  • Only valid and unexpired licenses will be accepted for authentication

 

Travel Document

  • Duly filled-out Travel Document application form
  • Two (2) recent ID pictures (2×2 picture or passport size)
  • Affidavit applying for issuance of Travel Document, stating reason for loss of passport, personal experience requiring the Embassy’s assistance, reason for indigency (i.e. financial incapacity)
  • Photocopy of passport, or any valid Philippine government-issued ID, or NBI clearance, if any is available

Philippine Visa (KD 13.00 for Single-entry Tourist Visa; KD 39.00 for multiple-entry visa)

  • Duly filled-out visa application form (visa application form)
  • One recent ID picture (2×2 picture or passport size)
  • Copy of passport and valid Kuwait visa (present ORIGINAL PASSPORT for visa stamping)
  • Copy of old/previous Philippine visas, if any
  • Company Letter: must be duly signed by the Letter from company in Kuwait stating that it has no objection to the travel of its employee to the Philippines and/or authorizing such employee to attend a particular meeting in the Philippines; the letter should also state the employees’ duties and responsibility at the company, his position and salary
  • Confirmed ticket in going to and from the Philippines
  • For first-time applicants, PERSONAL APPEARANCE is required
  • For multiple-entry visa, a DFA-authenticated marriage certificate or Report of Marriage to the Embassy is required in addition to the above-mentioned requirements
  • A multiple-entry visa is issued after about three (3) to six (6) months upon prior approval from DFA

Business/Recruitment Visa Requirements

  • Passport valid for at least six (6) months beyond intended length of stay in the Philippines.
  • Photocopy of passport
  • Duly accomplished visa application form (download application form)
  • Two colored photos
  • Request letter from sponsor/employer, including details of visa applicant and purpose of travel (for non-Kuwaitis)
  • Copy of round-trip airplane ticket
  • Personal appearance (upon request)

If the purpose of the visit is for recruitment of Filipino workers, applicant must submit the following:

  • Copy of the Special Power of Attorney verified by the Office of the Labor Attache
  • Copy of demand letter stating the number of workers to be hired, their positions, and salaries
  • Copy of approved block visa from the Ministry of Foreign Affairs
  • Fees: KD 13.000 (single entry/three months); KD 26.000 (multiple entry/six months)
    KD 39.000 (multiple entry/one year)


Requirements for Student Visa
 (KD 65.00)

  • Authenticated secondary diploma and transcript of records
  • Personal History Statement (PHS) to be filled up in quadruplicate
  • Affidavit of support from parents
  • Police clearance
  • Medical examination report with life-size chest x-ray film
  • Certificate of good conduct
  • Duly filled up application form
